Job Description:

Recruits and manages territory growth and development, generating leads to recruit additional providers and outpatient entities into CMI initiatives. Pitches and presents to leadership and providers, facilitating market expansion and onboarding to mission initiatives. Utilizes clinical best practices integrated into the mission to drive core CMI metrics and sustain CMI initiatives. Tracks referrals from territory sent for Spiritual Counseling and ensures the Spiritual Care Plan is returned to the provider. Maintains accurate documentation of spiritual care referrals and a network of spiritual support for patients. Provides training and ongoing support for providers and staff to conduct spiritual assessment screenings. Assists in the development and ongoing support of Spiritual Care Teams, mentoring and supporting Spiritual Care Coordinators. Participates in Spiritual Care Team Huddles as indicated, collecting and reporting stories that highlight mission integration impact. Maintains a log of practice and outpatient entity visits by date, adhering to required metric accountabilities and data collection. Establishes relationships with diverse community faith leaders to create a network of spiritual support for patients. Liaises with hospital chaplains for continuity of patient’s spiritual care on admission and transition of care to and from the hospital. Other duties as assigned.Knowledge, Skills, and Abilities:

Education:

• Bachelor’s [Required]

Field of Study:
 
• in business, healthcare or theological studies 
• One or more units of CPE at a center accredited by ACPE, Inc. OR completion of the AH Spiritual Integration and Support Education Program [Required]

Work Experience:

• 3+ business development, territory management and/or training experience [Required]
• 5+ experience in the hospital and/or outpatient clinical setting [Required]
• 5+ experience providing outpatient practice management support [Preferred]

Additional Information:
 
• One or more units of CPE at a center accredited by ACPE, Inc. OR completion of the AH Spiritual Integration and Support Education Program [Required]
